Our team-based approach:
We take a team-based approach to each member’s care and customize our services around your needs. We’re professionals from a variety of backgrounds – doctors, nurses, social workers, therapists, coaches, and more – all working toward the common goal of helping you achieve a better life filled with belonging, love, and purpose. You’ll have your own physician or nurse practitioner overseeing your medical care, working with our broader team of support staff to tailor care to your needs.

We work with each member to connect you to the services you need in the community, whether it’s additional healthcare or social support. Our community health coordinators are a diverse team of locals who know the community. We partner with other community-based organizations across our region to help you access the social services and medical and behavioral health care that works best for you.
