How does it work?
Here’s our care model to help members of your community move beyond addiction.
Our medical providers may prescribe and manage buprenorphine/naloxone (Suboxone) or other medication to help members feel well
Group therapy sessions with our licensed professionals, in person or virtually
A caring team of doctors, counselors, and support staff working with members via phone, text, or email

Our services
Opioid use disorder (OUD) treatment
We work with members to find the right combination of counseling, community support, and life-saving medication.
Alcohol use disorder (AUD) treatment
Members join a peer counseling group and work with a care team who understands their journey. If medication is part of their recovery plan, we can help with that too.
Mental health care
We’ve got members covered with help managing co-occurring conditions.
Responsive care
We’re here to treat the whole person. From STI care to pain and fevers, we’ll respond to our members’ health needs.
